To establish a new prize, download and complete the Proposal to Create a New Established Prize form.

This form should be completed consistent with the Guidelines for Creating Faculty and Departmental ‘Established’ Awards in the VCAC Guidelines for the Establishment and Variation of Awards, Prizes and Scholarships - click here.

Return the completed form to the Scholarships & Prizes Office so that we can formerly establish your prize in FlexSIS.
